Background
The consultant will provide technical support to the Health Systems Insight (formerly ThinkWell Institute) team as part of the Strategic Purchasing for Primary Health Care (SP4PHC) project. SP4PHC is a Gates Foundation funded program that supports countries to improve how funds are allocated to healthcare providers to achieve the goal of universal health coverage.
In Indonesia, SP4PHC is working with Ministry of Health (MOH) to support the MOH’s Health Transformation agenda, specifically focusing on reforms around improving primary health care, health financing, and digital health across Indonesia. SP4PHC Indonesia is implementing three strategies, which aim to improve how JKN purchases PHC services, optimize flow of funds via districts to PHC providers, and foster integration of telemedicine into purchasing arrangements for primary health care services.
The Integrasi Layanan Primer (ILP) is the key vision for the Ministry of Health’s primary health care (PHC) reform in Indonesia, focused on a life-course, integrated care delivered through puskesmas-pustu-posyandu networks, and supported by a suite of health information, equipment, and health workforce strengthening interventions. The Gates Foundation has been supporting the MOH in developing and testing innovative approaches for ILP, and its rollout and tracking through multiple activities implemented by a set of local and international partners, covering digitalization and interoperability at facility and national levels, system design and planning, policymaking based on evidence, and financing for PHC.
In the three years since the launch of ILP, there has been significant progress in reaching local government areas and PHC facilities with all the comprehensive elements of ILP. Targets established by the MOH – such as those incorporated in a results-based loan for PHC strengthening from the Asian Development Bank (ADB) – suggest that by 2026, the percentage of puskesmas implementing ILP should reach 75%.1 There are related targets for puskesmas and labkesmas supplied with the right equipment, posyandu activated with the life-cycle approach, and community-level workers trained and activated, among others. Anecdotally, the achievement across Indonesia’s vast archipelago is variable2 – some areas set more ambitious targets, such as Jawa Tengah, which aimed at 100% of puskesmas implementing ILP by 2024. There is no single integrated data platform that captures what percentage of puskesmas are implementing ILP, and the level of achievement of other elements of ILP across all kota/kabupaten.
There is urgent need for the MOH to establish the current baseline for ILP achievement and performance by component of this reform at the district level and identify common barriers to implementation by kota/kabupaten (~regencies). The Health Systems Insight (HSI) team will then work with the MOH and partners to develop a Technical Acceleration Plan (TAP) to improve ILP implementation at the district level and cost this plan. Such targeting of central MOH efforts is also necessary given ongoing budget constraints with the implementation of blanket reductions in planned 2025 spending across ministries – with the MOH implementing an 18.6% cut. The policy impetus for such analysis is also provided by Inpres No. 5/2025, which requires acceleration of PHC transformation over 2025 to 2029
To support this work, HSI will recruit an analyst consultant for Integrasi Layanan Primer (ILP) Targeted Acceleration Plan (TAP). These include specific analyses focus on developing and utilizing the TAP. The consultant will provide technical and analytical support to HSI and the Ministry of Health (MOH) to inform the development of the TAP. Working under the direction of the SP4PHC Project Director, Senior Technical Advisor, and Senior Advisor for ILP, the consultant will help consolidate existing ILP data, contribute to analysis and validation of indicators, and prepare background materials that align with the MOH’s technical direction.
The Consultant will report to the SP4PHC Project Director. This is a part-time position is 24 hours per week and can work remotely with the SP4PHC team in Indonesia.

| 1. Subnational performance baseline by ILP component: Undertake an analytic exercise utilizing all available data on ILP components to identify the achievement baseline as of end-2025 by kota/kabupaten (N: 514), identifying status of rollout – expressed as much as possible in percentage achievement terms.
Under the supervision of Senior Advisor, the Analyst will conduct data collection and extraction from relevant sources and ensure data completeness and quality.

| 2. Kota/kabupaten barrier analysis and grouping: Using the performance measurement as well as consultations, coordinate analysis and group local government areas based on observed clustering around specific types of ILP rollout gaps that point to common underlying barriers.
The Analyst will run data queries and preparing grouped outputs for validation.

| 3. Develop the TAP for ILP rollout: Using the clustered lists of kota/kabupaten, along with an analysis of the engagement strategies employed by the MoH and its partners, develop a set of targeted local technical assistance and capacity-building activities. These activities will be tailored to address the specific barriers faced by each cluster.
The Analyst will assist the Senior Advisor to compile analytical findings and technical inputs to inform the draft TAP and coordinate with the MOH and TWI leads on framing recommendations and preparing the draft document.

| 4. Costing the TAP and projecting its impact on national targets. The TAP will be costed based on the solutions identified and their scope and coverage across kota/kabupaten clusters. This will also involve considering unit costs from existing PHC strengthening activities of the MOH and its partners, along with other relevant cost data to be identified. It is anticipated that the TAP will cover the period 2026-27, with the majority of activities scheduled to take place in earlier part of this time frame.
The Analyst will validate data inputs, and documenting preliminary assumptions used to estimate resource needs and potential contributions to national ILP targets.

Job Location: This part-time consultant position (Indonesian Nationals Only) who could remotely work with the SP4PHC Indonesia team.
Timeline: The contract lasts from December 1, 2025, to May 31, 2025. The maximum Level of Effort (LOE) is 24 hours per week, for a total of 144 hours for the duration of the consultancy.

QuAlifications
- Excellent written communications skills in English and Indonesian
- A minimum Master degree in health policy, health economics, bio-statistics, or relevant fields,
- Having 7+ years experience in health sector
- Familiarity and expertise on big data analysis, using STATA/ SPSS/ or R
- Experienced doing primary data collection at the district level
- Capacity to build research proposal, analytical plan, and report writing
- Strong interpersonal skills and cultural sensitivity to effectively communicate with government officials, development partners, other staff, and stakeholders.
- Ability to work remote effectively; and be proactive, flexible, conscientious, and responsive.
